Who We Help

Our services help adults 18 and older who need assistance with a health challenge, including those who are:

  • Recovering from injury or surgery
  • Experiencing dementia-related symptoms
  • Undergoing treatment for cancer or other diseases
  • Suffering from a chronic illness like COPD or diabetes
  • Needing assistance coordinating care needs
  • Aging in place and wish to maintain independence

How We Help

FirstLight’s Healthcare Solutions program includes a variety of services.

A nurse
Skilled Nursing (Check with your local office for Skilled Nursing services availability)

These services help clients and families manage their overall health and wellness and increase positive outcomes. They may also help reduce unnecessary physician visits and hospitalizations.

Services are provided by a Registered Nurse (RN) or other licensed and/or trained staff delegated and supervised by the RN, and may include:

  • Medication management, including set-up for an easy-to-follow regimen, coordination with ordering physician, and administration when extra help is needed
  • Diabetic care, including blood sugar testing
  • Wound care and dressing changes
  • Bowel and bladder programs
  • Catheter and ostomy care
  • Tube, line and drain care
  • Oxygen / Breathing treatments
  • Infusion therapy

Care Management

These activities can help address and manage multiple health issues at once. Providing the greatest level of care at home and promoting long-term wellness allows clients to age in place, wherever home may be.

Services include:

  • Monitoring and coordination of medical care
  • Arranging for community resource assistance
  • Monitoring the home for safety concerns
  • Assisting with hospital-to-home transitions
