Person-First Dementia Care
Understanding that dementia is different for each person who suffers from it, San Rafael’s FirstLight Home Care has developed a Dementia Care Program and a home care assistance that is one of a kind. We build our care plan around the individual person’s needs, not the progression of the disease. We look past the person as they are today and incorporate who they were prior to developing dementia. Our approach is to personalize our home care assistance and draw our patient into the program, with the goal of providing them with a purposeful and engaged life.
We provide clients with a booklet called “Who I Am.” This booklet contains much more than basic medical history. We want to know the things that make your loved one happy and what makes them frustrated. We want to know what gives them a sense of accomplishment and what makes them feel like laughing. Our coordinators use this booklet to create a dementia care plan that is entirely specific to the individual. One way we do this is to bring you and your family into care planning, even our caregiver selection. As your loved one’s’ needs evolve, our home care assistance plan for our Dementia Care Program will as well, and we evaluate our program with satisfaction surveys, client and caregiver focus groups, and advisory councils.
Care Coordinators and Caregivers then use this information to build a specific and individualized plan for your loved one’s daily care.
