Person-First Dementia Care
The home care staff of FirstLight understands that each individual affected by dementia or Alzheimer’s is still a unique individual, regardless of their state of dementia or Alzheimer’s. Our Dementia Care and Home Care Assistance Program can provide the highest level of care each patient. We find that recognizing the individual, in addition to the disease, allows us to build an effective program to help our clients to live life with purpose.
Individuals and their families in our Dementia Care & Alzheimer Care Program are asked to fill our a “Who I Am” booklet. The questions that we ask allows us to identify and appreciate certain things about your loved one, such as:
- What makes you happy?
- Feel accomplished?
- What distresses or frustrates you?
Care Coordinators and Caregivers then use this information to build a specific and individualized plan for your loved one’s daily care.
Family members are always included in the caregiver selection process and personalized consultations. We value your input and use satisfaction surveys, suggestions, client/caregiver focus groups, and consultative meetings as a way to continuously improve our services.
