What is dementia?

It is a decline in the brain function over time caused by brain disease or injury. The individual will act, remember, behave, and talk differently because, they are who they are.

Changes in personality, memory challenges, and impaired reasoning are a few first signs someone is struggling with the disorder.

To get to know your loved one better, we have a “Who I Am” booklet to answer more about who they are. Our goal is to understand their needs and individualize their plans to give them the best of quality daily care. We ask questions more specific regarding your loved one, such as:

  • What makes you happy?
  • What distresses or frustrates you?
  • What makes you feel accomplished?

By having more definitive information about your loved one, our Care Coordinators and Caregivers will act as a team with you, to build a more specific and individualized daily Alzheimer’s care plan through our Culture of Care and evaluate its effectiveness for your family member with dementia helping them live a richer and fuller life through:

  • Client Satisfaction Surveys
  • Client and Caregiver Focus Groups
  • Advisory Councils
