We have you and your loved one complete a “Who I Am” booklet, which gives our caregivers a complete understanding of the person, more than just a basic history. Some of the things we want to know are:

- What makes you feel accomplished or gives you purpose?
- What are some thing that distress or frustrate you?
We use this information to build a care plan that provides our caregivers with a personalized and detailed guide for daily assistance.
