Participants are paired with a dedicated care manager or care navigator who helps coordinate services, answer questions, and create a personalized dementia care plan tailored to the individual’s needs.
Caregiver Education and Support
Family caregivers receive education and training to understand dementia better, manage behavioral changes, and provide effective support for their loved one.
Access to Community Resources
The GUIDE Program helps connect families with valuable local and community-based services, including transportation assistance, meal delivery programs, and additional support resources.
Respite Care Benefits
One of the most valuable features of the GUIDE Model Program is access to respite care services. Eligible participants may receive up to $2,500 annually in respite care support, helping family caregivers take necessary breaks while ensuring their loved one continues receiving compassionate care.
Respite care services may be provided in:
- The individual’s home
- Independent living communities
- Assisted living facilities
- Memory care communities
Who Qualifies for the GUIDE Model Program?
You may qualify for the Medicare GUIDE Program if:
- You have a diagnosis of Alzheimer’s disease or another form of dementia
- You are enrolled in Traditional Medicare Parts A and B
- You live at home or in an independent living, assisted living, or memory care community
- You are not currently residing in a long-term nursing home
- You are not enrolled in hospice or a PACE program
