If your loved one has been diagnosed with dementia and is covered by Traditional Medicare, there is a landmark new program that could change everything for your family — and most people have never heard of it.
The Medicare GUIDE Model (Guiding an Improved Dementia Experience) is a CMS Innovation Center program that provides comprehensive, coordinated dementia care at $0 out-of-pocket cost to eligible Medicare beneficiaries. FirstLight Home Care of Alameda is one of the few approved GUIDE program agencies in Alameda County and Contra Costa County.
What Does the GUIDE Program Provide?
Through our partnership with PocketRN, enrolled clients receive:
- A dedicated Nurse For Life — a registered nurse available 24/7 by phone or video
- Up to 72 hours of free respite care per year — fully covered by Medicare, hours reset every July 1st
- Medication management — clinical oversight of prescriptions and interactions
- Care coordination — a navigator who works with physicians and specialists
- Caregiver education and support — training on dementia progression and management
Who Qualifies?
Your loved one must have Traditional Medicare (Parts A and B), a documented dementia diagnosis, and be living at home or in an assisted living community. Medicare Advantage plans are not currently eligible.
The July 1st Reset Advantage
The 72 hours of respite care reset every July 1st regardless of enrollment date. A family enrolling in October receives their full 72 hours immediately, then another 72 hours on July 1st — potentially accessing 144 hours in their first 12 months.
