Do you know… Brevard County has approximately 18,000 residents aged 65 and older that are living with Dementia?
Providing care for a loved one with dementia can be one of the most challenging responsibilities a family may face. It requires significant emotional strength, physical effort, and constant attention, which can place a heavy strain on caregivers over time, even causing “caregiver burnout”.

This letter provides general information about the CMS GUIDE (Guiding an Improved Dementia Experience) Model, a nationwide initiative known as the GUIDE Program which is offering up to *$2,500.00 of annual respite care and other valuable resources at no cost to individuals living with dementia as well as the family members who care for them.
Here are the criteria to be eligible to receive these “no cost to you” Dementia services. The
patient:

1. Must have an Alzheimer’s or Dementia Diagnosis.
2. Must have traditional Medicare (Parts A and B) and live at home or in a community such
   as: Independent Living, Assisted Care Facility, or a Memory Care Facility (no long-term
   nursing home patients).

If the above criteria are met and you would like to know more about the no-cost services, please contact our office today at 321-221-7099. If you qualify to enroll in the GUIDE Program, you will receive *$2,500.00 in respite care yearly. This *$2,500.00 will be available yearly through 2032.
