FIRSTLIGHT HOME CARE PRIVACY PROTECTION POLICY FOR CLIENT AND FAMILY CONFIDENTIAL INFORMATION
In compliance with the law and FirstLight Home Care’s commitment to its clients and their families, it is the policy of our Agency to protect the privacy and confidential information of our clients and their families.
FirstLight will:
- Protect the confidentiality of Social Security numbers and nonpublic client medical information;
- Prohibit unlawful disclosure of Social Security numbers and nonpublic client medical information; and
- Limit access to Social Security numbers and nonpublic medical information.
FirstLight will not intentionally disclose any individually identifiable private medical or private personal information about our clients or former clients, except as permitted or required by law, or as authorized in writing by our clients or their authorized representative. Although complete security cannot be guaranteed, we have implemented procedures to minimize the risk of access or disclosure of private individually identifiable health and personal information. (Individually identifiable health information includes, but is not limited to, names, addresses, date of birth, physical or mental health or conditions, medical records including diagnostic tests, lab results and similar medical record documentation, health insurance identification numbers, Social Security numbers, driver’s license numbers, state identification cards, alien registration numbers, account numbers, credit or debit cards.)
FirstLight makes reasonable efforts to restrict access to confidential individually identifiable medical and personal information to those employees who need to know that information in order to provide services to our clients. We make reasonable efforts to train our employees to protect the privacy of our clients. We maintain physical, electronic, and procedural safeguards that comply with federal and state law to guard our clients’ medical and personal information.
FirstLight will destroy, erase or make unreadable individually identifiable health and personal information prior to disposal. FirstLight may contract with a business that disposes of confidential personal and health information. Client information is only destroyed after being retained for seven (7) years after the client ceases services provided by FirstLight.
All complaints regarding potential violations of this policy will be investigated and FirstLight will take appropriate action including disciplinary measures up to and including discharge for noncompliance with this Policy.
