At FirstLight Home Care of West Suburban Boston, we believe that exceptional care begins with knowledgeable, compassionate caregivers. On June 18, over 40 of our professional caregivers participated in a special training session, “Caring for Patients and Families with Advanced Dementia at End-of-Life,” presented by Alixis Van Horn, APRN-C, ACHPN of Good Shepherd Community Care. The session was held both in person at our Needham, MA office and remotely, reflecting our ongoing commitment to caregiver education and to providing families with the highest quality home care possible.
This training gave our team practical guidance for supporting individuals with advanced dementia and the loved ones who care for them. Topics included the stages of dementia progression, common signs of decline, end-of-life symptoms, frequent complications such as infections, falls, swallowing difficulties, and skin breakdown, and the importance of recognizing that behavior is often a form of communication.
Caregivers also learned communication approaches such as validation, distraction, environmental adjustments, and simple, non-confrontational language to reduce distress and improve comfort.
Just as important, the session focused on how to support families during a difficult and emotional time. Attendees explored strategies for educating families, responding to resistance or unrealistic expectations, and recognizing the grief, stress, and ambiguous loss that often accompany advanced dementia. The training also reinforced the importance of creating a familiar, calming environment, maintaining preferred routines, minimizing overstimulation, and using compassionate presence, eye contact, and a gentle tone to preserve dignity at every stage.
For care managers, discharge planners, and other referral sources, this kind of ongoing training is an important indicator of quality. It shows that our caregivers are being prepared to handle complex situations with clinical awareness, emotional sensitivity, and a strong understanding of hospice-informed care principles. When families are facing advanced dementia, choosing a provider with trained caregivers can make a meaningful difference in comfort, communication, and overall peace of mind.
